Accommodation Letter Service
Service Description
A customized clinical letter supporting reasonable workplace or school accommodations related to a mental health condition. Includes evaluation of symptoms and functional limitations, with clinically appropriate recommendations such as flexible scheduling, additional breaks, reduced-distraction settings, testing accommodations, or remote/hybrid participation. Approval of accommodations is determined by the employer or educational institution.
Cancellation Policy
Appointments canceled or rescheduled less than 24 hours before the scheduled appointment time are subject to a late cancellation fee of up to 50% of the scheduled service fee. No-shows may also be subject to this fee. Exceptions may be made at the practice's discretion for emergencies or other extenuating circumstances.
